Student Employee Enrollment Application

To Enroll a Child:

Fill out the Name, Phone Number and Email of the Parent/Sponsor/Guardian of the Child or Children. Then fill out the form below for as many children as you are sponsoring.

Include:

1.) Name
2.) Date of Birth
3.) Participation Level Desired
4.) Duration Desired
5.) Take Home Hourly Pay Rate

Choose the button that corresponds to how many children you wish to sign up

Ex: I have 4 children I wish to sign up, I would therefore choose the "Four Children" button. If you have more than nine, submit mulitple forms.
