To Enroll a Child:

Fill out the Name, Phone Number and Email of the Parent/Sponsor/Guardian of the Children. Then fill out the form below for as many children as you are sponsoring.

Include:

1.) Name
2.) Date of Birth
3.) Participation Level Desired
4.) Duration Desired
5.) Take Home Hourly Pay Rate

Level is a number from 250 to 800 in 25 increments (250, 275, 300...750, 775, 800). This defines the participation level as described in the "Basic Program" document.

Duration is a number from 6 to 108 in 6 increments (6, 12, 18, 24...90, 96, 102, 108). This defines the number of months of participation as described in the "Basic Program" document.

Take Home Pay should be an amount between $2.25 to $13.00 (higher amounts decrease the IRA value).
